I'm either in remission or the disease is considered mildly active at this time - my symptoms are generally manageable and rarely cause me to miss work or fall behind on other personal obligations

I've been hospitalized due to my illness in the last 6 months and/or experience severe and persistent symptoms that greatly impact my ability to function on a daily or near-daily basis

Yes, and they are fully supportive of whatever decision I make to improve my health
